For parents/guardians who feel that their child may have a disability that impacts their education and would like to request a potential evaluation by the child study team for special education and related services, please email your request for your child to be evaluated to referrals@madisonnjps.org. 

When your request is received, a case manager will be assigned and will be in touch with you within 20 days to set up a planning meeting.  At this meeting, you will meet with the child study team and your child's teacher to discuss your concerns.  The case manager will explain the next steps to you from there.
