Registration for the 2025 Spring Athletic season is open. Any student interested in playing a spring sport must register, including those that played in the fall and/or winter. Please utilize this link to create a family registration account with rSchool Activity Registration. Student ID numbers are needed to register and can be found on the summary page of your child’s Genesis account.
Physical examinations that athletes used for the fall and/or winter seasons are not necessarily valid for the spring season and may need to be updated. The date of your athlete’s physical must be within 365 days of the start of the first practice for the sport. For a point of reference, any student planning on playing a spring sport must have a valid physical with a date of 3/10/24 or later.
The History Form, Physical Examination Form, and Medical Eligibility Form should be completed by the healthcare provider who completes the PPE. The completed Medical Eligibility Form is required to be submitted by the school at the time of registration. The Medical Eligibility Form provides space for the healthcare provider to share relevant health information with the school if necessary and includes a recommendation regarding the student’s ability to participate in athletics. It is important to note that the PPE must be conducted by a licensed physician, advanced practice nurse, or physician assistant who has completed the student athlete cardiac assessment professional development module developed by the NJDOE. The Health History Update Questionnaire is required to be completed by the parent/guardian as part of the registration process.
All paperwork must be electronically scanned and uploaded into your account. Cell phone pictures will not be accepted. As a safeguard, we recommend submitting all original physical paperwork to Nurse Fastiggi. The deadline to register and ensure that students are cleared by our school physician is Friday, February 28th.

All spring sports will have practices and games during the Madison School District’s spring break. Please coordinate your plans around these dates.
